First session questions

An overview of the questions I may ask in the first session. You do not have to answer any question you feel is uncomfortable, this is not an interrogation only an exercise in understanding your situation and physical and emotional world. Technology Questions Computers use programs such as a web browser, email programs and other software. There is a need to protect our computers and software from Internet threats, including, viruses, Trojan horses worms and spyware. Do you feel confident using the technology involved to access therapy over the Internet? Do you know what an anti-virus and firewall program are and do you use them to protect your computer? Do you keep the operating system and other programs updated? Does any person other than you have access to your account on the computer or any device you use for online therapy? Do you have a strong password protecting your personal account that is hard to guess and does anyone else know it? Do you feel confident in protecting your Skype account password and other account information from friends’ partners and family? Please be aware that if you have poor hearing you may not realise how loud the speakers on your computer or other device may be, that's why I normally suggest using a headset for security and privacy. General Health Questions I need to understand your psychological needs and any physical problems that have an emotional or physical impact on your psychological wellbeing. The mind and body are one entity, physical issues will affect the psychological part of you and vice versa, any physical problem such as an over or under active thyroid or diabetes can have an influence on how you think and feel, problems such as constant pain can affect sleep patterns and impact on depression and other concerns. How is your overall physical health and do you have any concerns? Have you had any tests for thyroid disease or diabetes? Have you had a physical check up with your Doctor in the last 12 months? Do you have an on-going physical problem that relates to: heart problems, high or low blood pressure? Do you suffer from epilepsy? Do you take any prescribed medication? Do you have any constant pain in your body? Do have problems sleeping? Do you take recreational drugs such as cannabis or cocaine or any others? Have you ever taken cannabis or cocaine or any other drugs? Do you drink alcohol and how often? Do you eat well, how good or bad is your diet? How much coffee and cola or energy drinks do you intake per day? Do you do any sports or exercise? How active are you in general? For females only Do you suffer from hormonal imbalances? Are you pregnant or could you be pregnant? Mental Health Questions Have you ever been diagnosed with a serious mental health problem such as bipolar disorder or any psychological disorder by a mental health professional? Have you been diagnosed with a depressive illness? Does anyone in your family have depression or any other psychological diagnosed problems? Have you had any serious thoughts about killing yourself now or in the past? Do you self-harm or have you done so in the past? Have you ever been treated for or had an issue with alcohol or drug addiction? Do you have any phobias? Do you experience panic attacks? Is there anything I should know about your mental health that you have not told me?
